It emails you a notification, the notification sits between two newsletters, and by the time someone replies, the person who filled it in has already called your competitor. The tool that collects a lead and the tool that works a lead shouldn’t be two tools with an inbox in between.

    One page people actually finish

    Ten field types: text, email, phone, number, date, dropdowns, checkboxes, choice lists — plus an “Other” option that stores what the person typed, not the word “Other”. One page, one submit, no friction.

    Every submission becomes a lead

    Name, contact, company and tags map straight onto the lead; everything else lands in its notes, readable. A default owner picks it up, a default pipeline is pre-selected for the convert. Nothing to connect, nothing to import.

    Spam never reaches you

    A honeypot, rate caps, a disposable-email blocklist and optional Cloudflare Turnstile — and filtered submissions still get a polite “received”, so bots learn nothing. Double submissions are caught too: one person, one lead.

    Consent that stands up

    A consent checkbox with your own terms, and what gets stored is evidence: the text version, the timestamp, the IP, the lawful basis — eight to choose from, including the GDPR Article 9 bases health practices need.

    A hosted page at your own address, an auto-resizing iframe, a script embed (inline, modal or slide-in), a React component, a Web Component. All of them render the same element in a Shadow DOM — your site’s styles never fight it, and a published edit updates every live copy.

    Your palette, your logo, six card styles, light and dark. The visitor-facing chrome speaks English, Ukrainian, Polish, German and Russian; your own questions stay exactly as you wrote them. Links can prefill fields — and a utm_source field captures the campaign onto the lead.

    Pick the fields, mark what’s required, add your consent text. The builder won’t let you publish a form that can’t capture a contact — a lead you can’t reach isn’t a lead.

    The hosted link goes in your bio and your ads; the embed goes on your site; the self-contained snapshot goes anywhere else. None of the snippets contains a secret — republish as often as you like, nothing breaks.

    Work the lead, not the inbox

    New submissions appear in Sales → Leads with the source attached. When the accountant asks, export the lot to CSV — it’s one click, and it’s in the audit log.

Do I need a developer to put it on my site?
No. The hosted page needs nothing at all — it’s a link. The embeds are copy-paste snippets; if you can paste a YouTube video, you can paste a form.
Can it do multi-page forms or conditional questions?
No — deliberately. A form here is one page a visitor finishes in a minute. When you want a one-question-at-a-time flow with completion analytics, that’s Cognitify Surveys — same builder family, same CRM underneath.
What happens to spam?
It’s filtered server-side — honeypot, rate caps, throwaway-email blocklist, optional Turnstile — and the bot still sees a normal “thank you”, so it can’t learn what tripped it. You see spam rows in the submissions table, not in your leads.
Where do submissions actually go?
Into your CRM as leads — with mapped fields, tags, consent and source. There is no “integration” because there is nothing to integrate.
